DECLARATION FOR ORIGINAL INVALID PENSION "A"

On this 10 day of July A.D., One Thousand Eight Hundred and Eighty-two
personally appeared before me, Thomas H. Dailey, Circuit Court Clerk, the same
being a Court of Record of the County and State aforesaid, WILLIAM J. ERVIN, a
resident of Goshen, County of Elkhart, State of Indiana, who being by me duly
sworn according to law, on his solemn oath, deposes as follows, to wit:
I am the identical WILLIAM J. ERVIN who was enrolled on the 9th day of June,
1862, in Company "F" of the 12 Reg't of Indiana Inf'y Vol's,
commanded by Captain Sam'l Boughter and I was honorably discharged at
Indianapolis, Ind, on the 20 day of June 1865, and my age is now 41 years.
While in the service aforesaid and in the line of my duty, I received the
following disability, to wit:

I claim pension on Deafness caused by exposure and fever with bad treatment in
Hospital at
