DECLARATION FOR ORIGINAL INVALID PENSION "A"

On this 10 day of July A.D., One Thousand Eight Hundred and Eighty-two personally appeared before me, Thomas H. Dailey, Circuit Court Clerk, the same being a Court of Record of the County and State aforesaid, WILLIAM J. ERVIN, a resident of Goshen, County of Elkhart, State of Indiana, who being by me duly sworn according to law, on his solemn oath, deposes as follows, to wit:

I am the identical WILLIAM J. ERVIN who was enrolled on the 9th day of June, 1862, in Company "F" of the 12 Reg't of Indiana Inf'y Vol's, commanded by Captain Sam'l Boughter and I was honorably discharged at Indianapolis, Ind, on the 20 day of June 1865, and my age is now 41 years.

While in the service aforesaid and in the line of my duty, I received the following disability, to wit:

About
Nov 1 1862, I was attacked with a lingering fever, contracted while doing guard duty on Talahatchie R.R.bridge, Tenn., producing entire deafness of left ear, with offensive discharges therefrom constantly. The drum of the ear being entirely gone. I was treated in Regimental Hospital in Dec & Nov 1862 by Drs. LOMAX and TAYLOR at Holly Springs, Miss., and at LaGrange, Tenn., Jan & Feb '62 by Dr. Holly of Ills.

I claim pension on Deafness caused by exposure and fever with bad treatment in Hospital at
Holly Springs, Miss.

I have resided at
Warsaw and Goshen, Indiana, and my occupation has been mechanic before my entry into the service aforesaid. I was of good, sound physical health being at enrollment a Farmer, and I am now partially disabled from obtaining my subsistence by manual labor by reason of my disabilities above stated, received in the service of the United States, and I make this Declaration for the purpose of being placed on the Invalid Pension Roll of the United States....My Post Office address is Goshen, County of Elkhart, State of Indiana, Box 1848.
