DECLARATION FOR PENSION

State of Ohio
County of Butler

On the 18th of January A.D., 1915, personally appeared before me, a Notary Public within and for the county and State aforesaid, WILLIAM J. ERVIN, who being duly sworn according to law, declares that he is 73 years of age, and a resident of Hamilton, County of Butler, State of Ohio, and that he is the identical person who was enrolled under the name of William J. ERVIN on the 9th day of June 1862, as a Sergeant in Co. "F", 12th Regt. Indiana Infantry Volunteers in the service of the United States in the Civil War, and was honorably discharged at Washington, D.C., on the 8th day of June 1865.

That his personal description at enlistment was as follows: Height: 5 feet 8 inches; complexion: light; color of eyes: blue; color of hair: dark; that his occupation was Farmer; that he was born
September 15, 1841, at Kosciusko County, Indiana.

That he was originally pensioned under the general laws for "disease of left ear and resulting total deafness of same, result of typhoid fever". That his claim filed about August 1913, was rejected on the ground that he was not unable to perform manual labor by reason of same. That he at that time filed affidavit of his physician, Dr. Wm. Huston, showing total disability but that it was not considered nor was he ordered before a medical board for examination. He asks a reconsideration of his former claim and allowance from date of filing said claim. That the ear trouble also effects his throat and his disability is total.

That his Post Office address is
241 North Eleventh Street, Hamilton, County of Butler, State of Ohio.
